Name Change Certificate would be issued on the basis of particulars of Indian passport. One may apply for Name Change Certificate, only if the name has been changed in the passport.

Check-List of Documents:-

1. Duly filled in Miscellaneous application form

2. Self-attested copy of Indian passport containing the current name(Front and last page)

3. Self-attested copies of other Indian passports containing previous names (Front and last page)

4. Copy of the Visa/Resident Permit

5. City Registration (Meldebescheinigung)

SUBMISSION OF APPLICATION: Application along with other required documents mentioned above may be submitted in person on any working day between 9.30 hrs and 12.30 hrs at the Consular Wing counter.
